Dawn Marie Formanek, age 64 of La Crosse, Wisconsin passed away at Lakeview Healthcare in West Salem, Wisconsin on May 5, 2017. She was born in La Crosse on November 26, 1952 to Tom and Colleen (Adams) Formanek. Following graduation from La Crosse Logan High School, Dawn was employed at Gundersen Lutheran in medical records until her retirement.

Dawn in survived by her brother, Randy Formanek; aunts, Jane Mosher of Viroqua, and Marlyn Tovar of La Crosse; niece, Shaylee Formanek; and nephews, Tanner and Mitch Formanek. She was preceded in death by her parents; aunts, Bonnie Lovejoy and Beaulah Connors; and uncles, Beauford Adams, Richard Adams, Robert Adams, and Johnny Adams. Friends and family are invited to a memorial visitation on Thursday, April 13, 2017 at Dickinson Family Funeral Home (401 Main Street, Onalaska) from 11:00 a.m. until 1:00 p.m.

A burial will take place at Oak Grove Cemetery in La Crosse at a later date.
